Health Net, Inc. (Health Net) is a managed care organization. The Company provides managed health care services through health plans and government-sponsored managed care plans. Its segments are Western Region Operations and Government Contracts. It provides and administers health benefits to approximately 6 million individuals across the United States through group, individual, Medicare, Medicaid, dual eligible, the United States Department of Defense (Department of Defense or DoD), including TRICARE, and the United States Department of Veterans Affairs programs. Through its subsidiaries, it also offers behavioral health, substance abuse and employee assistance programs, managed health care products related to prescription drugs, managed health care product coordination for multi-region employers, and administrative services for medical groups and self-funded benefits programs.

ProShares Ultra SmallCap600 (the Fund) seeks daily investment results that correspond to twice (200%) the daily performance of the S&P SmallCap 600 Index (the Index). The Fund intends to invest at least 80% of its net assets, including any borrowings for investment purposes, under normal circumstances, to equity securities contained in the Index and/or financial instruments that, in combination, have similar economic characteristics. The Fund also intends to invest assets not invested in financial instruments, in debt instruments and/or money market instruments. The Fund intends to concentrate its investments in a particular industry or group of industries to approximately the same extent as the Index is so concentrated. ProShare Advisors LLC serves as the investment advisor to the Fund.
