Lexeo Therapeutics NASDAQ: LXEO outlined its approach to evaluating LX2020, an investigational AAVrh10-based gene therapy for PKP2-associated arrhythmogenic cardiomyopathy, or ACM, ahead of an expected clinical and regulatory update later this quarter.
CEO Nolan Townsend said PKP2 ACM is a serious, progressive genetic heart disease with no approved therapies designed to modify its course. The company estimates that about 60,000 people in the United States are affected and characterized the condition as an underappreciated cardiovascular rare-disease opportunity.
LX2020 is designed to deliver a full-length functional PKP2 gene to cardiomyocytes. PKP2 is a component of desmosomes, structures that help maintain connections between heart cells. Lexeo said PKP2 deficiency can contribute to electrical instability, mechanical dysfunction and progressive cardiac injury.
Focus on arrhythmias and NSVT
Chief Medical Officer Nani Bhalla said Lexeo will assess treatment response across three areas: arrhythmic activity; myocardial structure and function; and patient and clinical burden, including symptoms, treatment escalation, hospitalizations, ablations and implantable cardioverter-defibrillator, or ICD, shocks.
For the upcoming data readout, the company said nonsustained ventricular tachycardia, or NSVT, will be an important lens for assessing clinically relevant arrhythmic activity, alongside premature ventricular contraction, or PVC, burden, safety and other patient-level data.
Bhalla described PVCs as a quantifiable measure of ventricular ectopic activity that can fluctuate with factors including exercise, stress and caffeine intake. NSVT, by comparison, consists of three or more consecutive ventricular beats at more than 100 beats per minute that end spontaneously within 30 seconds under the guideline-based definition Lexeo plans to use going forward.
The company previously reported “total ventricular runs,” defined as three or more consecutive ventricular beats at any rate, in its January 2026 data cut. It said the shift to guideline-defined NSVT does not change the underlying electrocardiogram data, but provides what it considers a more clinically meaningful classification of events.
Lexeo said every participant had at least one qualifying guideline-defined NSVT event at baseline. Townsend said the company expects to present time-course data, including a “spaghetti plot,” when it releases additional results this quarter.
Clinical perspective on disease management
Alessio Gasperetti, a clinical electrophysiologist at Cleveland Clinic, said clinicians treating ACM patients must assess both their current arrhythmia risk and their likely disease progression over time. Sudden cardiac death from ventricular arrhythmias is a principal concern in earlier disease stages, while later progression can lead to advanced heart failure and potentially heart transplantation, he said.
Gasperetti said both PVC burden and NSVT are important measures in risk stratification, but NSVT generally has greater prognostic significance. He noted that even one or two NSVT episodes on Holter monitoring can change a patient’s arrhythmic-risk category and prompt consideration of an ICD independent of PVC burden.
“Just because the number is low doesn't mean that the effect size is low,” Gasperetti said regarding low-frequency NSVT events.
He said the ideal result would be for NSVT to disappear, while acknowledging that complete elimination may not be achievable for all patients over a short period or with one intervention. Gasperetti cited flecainide data showing roughly a 30% reduction in NSVT burden and said having approximately one-third to 40% of previously affected patients become NSVT-free, with a 30% burden reduction among those who remain affected, would be a reasonable therapeutic outcome.
Development and commercial considerations
Townsend said Lexeo believes a benefit beyond the 20% to 30% relative NSVT-risk reduction described for current care, potentially in the 35% to 40% range and accompanied by favorable safety, could be meaningful for patients and physicians. He added that the company wants to see an effect that is sustained and potentially deepens over time, as the biological effects of a gene therapy may take time to translate into clinical benefit.
In response to analyst questions, Lexeo said it does not believe NSVT burden will impede enrollment in a future registrational trial. Bhalla said the company believes 85% to 90% of the relevant patient population has either PVCs and NSVT or low PVC burden with primarily NSVT. Townsend also noted that many trial participants have existing ICDs, and NSVT can be a factor in ICD implantation.
The company did not disclose the planned design of a potential registrational endpoint, saying that discussion remains premature. Townsend said NSVT and PVCs are expected to be part of the broader picture, while Lexeo continues discussions with regulators.
Townsend said Lexeo views LX2020 as a potential multibillion-dollar opportunity based on the addressable population and the potential value of a one-time disease-modifying therapy. He said patients are concentrated at specialized heart-failure treatment centers and that Lexeo’s manufacturing platform is intended to support scalable supply. The company said the commercial opportunity ultimately depends on demonstrating that LX2020 can meaningfully alter the course of disease.
About Lexeo Therapeutics (NASDAQ:LXEO)
Lexeo Therapeutics, Inc is a clinical-stage biotechnology company developing genetic medicines for genetically defined cardiovascular and central nervous system diseases. The company uses adeno-associated virus (AAV) gene-delivery technologies to develop treatments intended to address the underlying causes of disease rather than only manage symptoms.
Lexeo's pipeline includes investigational programs for conditions such as Friedreich ataxia-associated cardiomyopathy, a serious heart complication of Friedreich ataxia, and genetic forms of neurodegenerative disease.
