Satellos Bioscience NASDAQ: MSLE said it will use MRI-measured muscle fat fraction as the primary efficacy endpoint in its Phase II BASECAMP study of forazapadin for boys living with Duchenne muscular dystrophy, following a Type C interaction with the U.S. Food and Drug Administration.
The company expects to report top-line results from the study’s 12-week placebo-controlled period in the first quarter of 2027. Satellos believes positive fat-fraction data that correlate with clinical function could support a potential accelerated approval pathway, subject to further data and regulatory review.
“We have selected fat fraction alongside safety and tolerability, as we believe it is an objective and potentially clinically meaningful measure of forazapadin’s effect in Duchenne,” said Frank Gleeson, Satellos’ co-founder and chief executive officer.
Study to Assess Muscle Composition and Function
BASECAMP will measure changes in fat fraction in the vastus lateralis, a thigh muscle. Chief Medical Officer Dr. Wildon Farwell said fat fraction rises as damaged muscle is replaced by fat and that natural history studies have associated increases with worsening function and earlier loss of ambulation.
At 12 weeks, Satellos plans to compare MRI results for participants receiving forazapadin with those receiving placebo. The analysis is expected to include all randomized participants and assess both the 60-milligram and 120-milligram treatment arms, along with safety and tolerability.
The company also plans to report available data on timed function tests, strength and biomarkers, as well as six-month data from participants who have reached that milestone. Functional assessments include stride velocity, time to rise, a four-stair climb and dynamometry. Farwell said the study will also evaluate a regenerative index using muscle biopsies.
Farwell said the FDA discussion addressed the relationship between fat fraction and clinical endpoints without specifying a required degree of change. He said the agency did not express concern about BASECAMP’s size or conduct, but did not describe an FDA determination that the study would be sufficient for approval.
In response to an analyst’s question about a roughly 1.3% difference between treatment and placebo, Farwell said that range was consistent with the company’s thinking and could allow the study to show statistical significance. He said a statistically significant difference in fat fraction correlated with clinical function would provide the clearest potential path to accelerated approval, in the company’s view.
Earlier Study Offered Preliminary Support
Satellos pointed to results from TRAILHEAD, an open-label study in four adults with Duchenne, as preliminary support for its approach. After five months of treatment, mean biceps brachii fat fraction fell from 49.7% to 46%, an absolute reduction of 3.7 percentage points, Farwell said. Each participant showed a decline.
The reduction was accompanied by increased upper-limb total effort, maintained improvement in hand grip strength and generally stable upper-extremity function after six months of exposure, he said. Farwell cautioned that TRAILHEAD was small and was not designed to establish efficacy. Satellos expects 12-month data from its four participants later in 2026.
Gleeson said the company plans to provide guidance in the first quarter of 2027 on its intentions for a potential filing and commercialization after it has data to analyze.
