Concentra Group Holdings Parent NYSE: CON raised its 2026 revenue and adjusted earnings outlook after reporting third-quarter growth in patient visits, revenue per visit and profitability.

The occupational health provider reported revenue of $622.1 million, up 8.6% from $572.8 million a year earlier. Net income attributable to the company rose to $65.9 million, or $0.52 per share, from $48.3 million, or $0.38 per share. Adjusted earnings per share increased to $0.52 from $0.39.

Adjusted EBITDA rose 17.9% to $140.2 million, while the adjusted EBITDA margin expanded 178 basis points to 22.5%. Chief Financial Officer Tanner Newton attributed the year-over-year improvement to volume and rate growth, disciplined execution and operating efficiencies.

The call was the company’s first since Matt DiCanio became chief executive officer and Tanner Newton became CFO, both effective Nov. 1. Keith Newton, who moved to executive chairman, said he would continue working closely with the leadership team during the transition.

Visits and pricing drive growth

Occupational health center visits increased 4.9% to an average of more than 58,000 per day. Workers’ compensation visits per day rose 4.2%, while employer services visits increased 5.8%.

DiCanio said growth was broad-based across all six regions and most end markets. He attributed the performance to a combination of favorable labor conditions in many blue-collar sectors and continued market share gains. Hiring-related services, including drug screens and physicals, were particularly strong among transportation and logistics, manufacturing and construction-oriented customers, he said.

In response to an analyst’s question, DiCanio said third-quarter visit growth was driven primarily by stronger initial injury visits, rather than by an increase in visits later in a patient’s treatment cycle. He added that customer retention remained at multi-year highs and year-to-date new business activity was ahead of 2025 levels.

Revenue per occupational health center visit increased 3.3%. Workers’ compensation revenue per visit rose 3.5%, largely reflecting fee schedule increases, while employer services revenue per visit rose 4%, helped by a greater mix of higher-revenue physicals relative to drug screens.

Occupational health center revenue increased 8.3% to $569.7 million. Onsite revenue rose 10% to $38.4 million, though Tanner Newton said organic growth in that business moderated as the acquired Pivot business entered the organic calculation following its acquisition anniversary.

Higher outlook, with 2027 rates still uncertain

Concentra raised its full-year revenue forecast to $2.36 billion-$2.38 billion, increasing the lower end by $35 million and the upper end by $5 million. It also raised its adjusted EBITDA outlook to $505 million-$510 million, up $20 million at the low end and $15 million at the high end.

The company left its free cash flow forecast of $220 million-$240 million and capital expenditure range of $70 million-$80 million unchanged. DiCanio said Concentra expects to provide formal 2027 guidance early next year.

For 2027, DiCanio said the company expects employer services rate growth to remain generally consistent with its long-term expectation of approximately 3%. Most states have not yet announced workers’ compensation fee schedules, he said, although management has no current reason to expect aggregate rate growth to differ materially from its long-term expectations.

Tanner Newton said he expects Concentra to sustain its current margin level, but probably not repeat the same degree of year-over-year margin expansion seen in 2026. He identified staffing efficiencies and potential additional center-level improvements as supporting factors.

Expansion and capital allocation

Concentra acquired four Minnesota medical centers and opened three newly developed centers during the quarter. Two more opened after quarter-end, and the company expects to open one additional center before year-end. Acquisitions and development are expected to add 15 centers in 2026, with low double-digit new center openings anticipated in 2027.

Operating cash flow fell to $51.9 million from $60.6 million a year earlier, primarily because of working capital timing and higher cash tax payments, Tanner Newton said. Free cash flow declined to $32.2 million from $40.2 million.

The company repurchased 1 million shares for $34.65 million during the quarter and had approximately $19 million remaining under its original $100 million authorization at Sept. 30. After quarter-end, the board approved a new $250 million repurchase program with no expiration date.

Concentra ended September with $1.52 billion in debt, $85.8 million in cash and a net leverage ratio of 2.90 times under its credit agreement. Tanner Newton said growth investments remain the company’s first capital allocation priority, with debt reduction and share repurchases also priorities as it works toward a long-term leverage target of 2.5 times.

DiCanio said separation activities from Select Medical were substantially complete and would conclude when the transition services agreement expires at the end of November.
