ClearPoint Neuro NASDAQ: CLPT reported third-quarter 2026 revenue growth of 36% as sales increased across its biologics and drug-delivery, neurosurgery navigation and therapy, and capital equipment and software businesses. Management said the company is investing in clinical support, laboratory capabilities and product development as it prepares for anticipated cell and gene therapy trial activity and potential commercial launches.
Total revenue rose to $12.1 million for the quarter ended Sept. 30, from $8.9 million a year earlier. Chief Financial Officer Danilo D'Alessandro said biologics and drug-delivery revenue increased 11% to $4.9 million, driven primarily by higher service revenue at ClearPoint Advanced Laboratories, or CAL.
Neurosurgery navigation revenue increased 59% to $5.4 million, reflecting sales of IRRAflow products acquired through the company’s fourth-quarter 2025 acquisition of IRRAS as well as growth in legacy neurosurgery products. Capital equipment and software revenue rose 68% to $1.7 million, aided by placements of ClearPoint navigation systems, Prism laser units and IRRAflow control units.
Margins and Spending
Third-quarter gross margin was 58%, down from 63% in the prior-year quarter. D'Alessandro attributed the decline primarily to overhead costs at the new CAL facility and additional amortization expense related to acquired intangible assets.
Research and development expense rose 16% to $4 million, mainly because of higher personnel costs. Sales and marketing expense increased 44% to $5.5 million, driven by staffing additions, travel associated with expanding the clinical and sales organization, and amortization expense. General and administrative expense increased 7% to $3.8 million.
Total operating expenses were $13.4 million, down from nearly $17 million in the second quarter, according to D'Alessandro. CEO Joe Burnett said sales and marketing expense declined more than 15% from the prior quarter following a commercial reorganization that prioritizes clinical case support.
The company ended the quarter with $46.2 million in cash and cash equivalents, compared with $45.9 million at the end of 2025. ClearPoint generated $22 million in net proceeds through its at-the-market equity offering program during the quarter, partially offset by $20.1 million of operational cash burn through the first nine months of 2026.
D'Alessandro said the company expects capital expenditures related to CAL over the next 12 months, while management seeks to continue reducing operating burn over the coming four to six quarters through operating leverage. He said first-half spending was elevated by IRRAS integration activities, including certain one-time costs.
Gene Therapy and Drug Delivery Plans
Burnett said ClearPoint continues to support partner uniQure on the device-related components of its biologics license application for AMT-130, an investigational therapy for Huntington’s disease. ClearPoint’s device documentation is on file with the FDA, he said.
If the FDA accepts uniQure’s application for review, ClearPoint expects to submit a companion 510(k) application for use of its device with AMT-130. Burnett said the company expects the drug and device regulatory processes to proceed in parallel, with the goal of simultaneous approvals.
During the question-and-answer session, Burnett said the company expects an FDA decision on whether to accept, reject or delay the AMT-130 submission within roughly the next month, based on management’s expectations. He said that, if the application is accepted and the process proceeds as planned, a final decision could occur six to eight months later.
Management also expects continued enrollment and initiation of multiple partner Phase III or confirmatory trials over the next year. Burnett said such studies can expand from fewer than 15 patients and a handful of sites in early-stage studies to as many as 200 patients across roughly 20 sites.
ClearPoint is expanding and training its clinical support organization in preparation for that potential activity. Burnett said hospitals increasingly view the company’s navigation systems as enabling technology for participating in cell and gene therapy trials, contributing to demand for capital equipment.
Four Growth Pillars
Burnett outlined four strategic areas: biologics and drug delivery, neurosurgery navigation, laser therapy and access, and intracranial fluid management.
- Navigation: The company reported its largest capital sales quarter to date and said it is developing a cranial-focused robotic platform for multi-trajectory procedures and drug-delivery workflows.
- CAL expansion: ClearPoint is adding capabilities at its Torrey Pines, California, laboratory, including PET and SPECT tracers, radiopharmaceuticals and histology services. Management said some investments will precede related revenue by several months.
- Spinal laser therapy: ClearPoint is participating in a five-year National Cancer Institute R01 grant led by Dartmouth Health to study MR-guided laser therapy for spinal metastasis using the Prism system. Prism is not currently available for spinal use. Burnett said the company estimates potential procedure revenue of about $13,000 to $20,000, or approximately $15,000 as a modeling assumption, if the program reaches commercialization.
- IRRAflow: The company is targeting five product and clinical milestones in 2027, including Shoreline software, an IRRAflow Dart catheter, a next-generation forward-tunneling catheter, ARCH trial results and pursuit of an FDA indication for subdural hematoma.
Looking toward 2027, Burnett said ClearPoint continues to view mid-teens base-business revenue growth as a reasonable expectation, though he emphasized the company had not yet issued formal guidance. Management expects to provide additional information in January after gaining more clarity on partner trial activity, the AMT-130 regulatory review, CAL installations and IRRAflow development milestones.
